We coordinate with the resident, family, key staff members, and physician to establish a unique, individualized care plan for each resident that is specific to them.
Medication Review
Each month, our pharmacist completes a medication review and may make recommendations for dose reductions where applicable.
Psychiatric
We work closely with a psychiatrist who makes monthly visits to all residents that benefit from those services.
Counseling Services
We contract with a counseling company to meet any mental health needs our residents may have.
Trust Fund Service
We offer a “bank” where residents are able to withdraw money for any personal needs.
Rehab to Home
Residents receive customized therapy services for medical conditions such as cardiac events, strokes, surgery, weakness, or other circumstances that require a little bit more attention after the hospital stay to get your back to your prior level of functioning before returning home.
- Speech Therapy: The assessment and treatment of communication problems and speech disorders.
- Occupational Therapy: Care that focuses on helping you perform daily tasks more easily.
- Physical Therapy: Rehabilitative care that focuses on improving movement, mobility, and function.
Extended Stay
When someone is no longer safe to live at home due to their medical condition, we offer the opportunity to transition to long term care. Whether it be a transition from therapy services or straight from home, we will help in education and preparation of this new chapter.
Respite Care
Caregiving is not an easy job for families. They are entitled to a well-deserved break or vacation. Allow us to take over care for a bit while during this reset. Ask us about payment assistance for this stay.
Hospice/Palliative Care
We contract with most hospices and provide palliative care for those who shift to end-of-life. We can help manage symptoms and provide 24-hour nursing assistance so family, friends, and visitors can live in the presence rather than focusing on care.
IVs, Pic lines, Tube feeding, Wound Care
Some maybe discharged from the hospital with a continuation of post-acute care. We can assist with the supervision and administration of these physician orders. We will communicate any complications to the physician for any change or updates to care.
Weight and Nutrition Monitoring
It is important that dietary restrictions, preferences, and weight is tracked to aide in the recovery process to improve health and stabilization.
X-ray, Lab, Dentistry, Podiatry, Physician Visits
These services are provided in-house to help limit medical appointments for our residents.
