As part of the discharge process from the hospital, a case manager will ask which facility records should be sent to. They will offer a list of facilities to choose from. Tours may be scheduled with different facilities to help answer any questions that may arise with the specific scenario. Each facility is unique and caters differently. There is also numerous resources that can be help with the choosing process under our resource tab. Once the facility is chosen, the referral process will begin. If a resident is coming from home, we can still initiate the referral process.
In the referral process, we recieve clinical and financial information to review to ensure compatibility with our services. We may reach out to family to discuss further needed information to further process the referral. Each case is unique which may require a different set of information. Once we have all the documentation that is needed to provide care, we approve the referral.
At that time, we set up a day and time for the admission. Once the resident has arrived, we begin our assessments which include nursing, therapy, activities preferences, dietary requests, and social services. We want to personalize the resident’s stay so we ask about their history, preferences, goals to make the most of the stay and have the best possible experience.
