Social Services

Once someone has achieved maximum potential and no longer require our services, our Social Worker will help transition to home.  We will initiate home health services and coordinate any special equipment needs to be delivered home. 

If there is a concern or question during the stay, we encourage feedback be given to our Social Worker to be addressed. 

Our Social Worker can also assist residents in obtaining services such as psychiatric, counseling, dental, optometry, or podiatry services.
